Grace Harbour provides compassionate, evidence-based mental health and substance use treatment for children, teens, adults, and families. We offer personalized care in the office, at school, and in the community, making it easier to access support in the setting that best meets each individual's needs.

Outpatient Therapy

Personalized mental health and substance use treatment for children, teens, adults, and families.

Our outpatient therapy services are available in the office, at school, or in the community based on each individual's needs. We work with children, teens, adults, and families to provide personalized mental health and substance use treatment where it is most accessible and effective.

Services may include:

  • Clinical assessments and service plan development

  • Individual, group, and family therapy

  • Substance use treatment and relapse prevention

  • Psychiatric medication evaluation and management

  • Care coordination and resource navigation

  • Crisis intervention and safety planning

SCHOOL-BASED SERVICES

We work closely with students, families, and school staff to provide confidential, consistent behavioral health services that support academic, social, and emotional well-being.

For students, we bring behavioral health services directly into the school setting, making it easier to access care while minimizing disruptions to the school day.

Our school-based services focus on:

  • Managing emotions, focus, and behavior

  • Coping with stress, grief, trauma, or substance use concerns

  • Building confidence and healthy peer relationships

  • Supporting academic and personal success

Services may include:

  • Clinical assessments and service plan development

  • Individual, group, and family therapy

  • Behavioral modification and coping skill development

  • Care coordination and resource navigation

  • Crisis intervention and safety planning

COMMUNITY SUPPORT
SERVICES

Community-based support that helps youth and families build skills, strengthen relationships, and achieve lasting success.

Community Support Services provide individualized, community-based behavioral health services for youth who need additional support beyond traditional outpatient treatment. Our team works alongside youth and caregivers to develop practical skills, strengthen family relationships, improve daily functioning, and connect families with community resources. Services are delivered in the home, school, and community, allowing support to occur where it is needed most.​

Our team can help with:

  • Developing individualized goals and service plans

  • Building coping, communication, and daily living skills

  • Improving family relationships and functioning

  • Preventing and managing behavioral health crises

  • Coordinating care and connecting families with community resources

  • Supporting success at home, in school, and in the community
